Dear Charles,

There are times a dental implant can be placed at the time of extraction. If there is inadequate bone to support a dental implant then a bone graft should be done either with implant placement or before the implant can be placed. If bone needs to be grafted prior to implant placement then 4-6 months time is needed for the bone to heal.

Bone grafting is needed in areas were there is a deficiency of bone. When a tooth is removed and there has been extensive destruction it is absolutely necessary. In some cases where there is sufficient bone the implant can be placed at the same time. In some cases dental professionals place bone at the time of implant placement. That is why a thorough exam is needed to evaluate this. Each case is different. The answer to your question is bone grafting does not always have to be performed after an extraction. Most cases that I see need to be grafted along with the extraction.

The definitive decision to graft a site is best made after the extraction has been performed. Extraction of large roots and difficult extractions can create a surgical defect requiring bone grafting prior to implant placement. The graft takes 6 months to mature. This information should be provided to you at your implant consultation appointment.
