Dear Celia:
The All-On-Four technique delivers a fixed (not removable by the patient) set of teeth the same day of the surgery which is made of pink acrylic resin and acrylic resin teeth. This choice of materials is kinder to the implants that still need to osseointegrate for a period of a few months in order for the procedure to be successful. During this time the bone fuses to the implants and the tissues heal from the surgery.
At the end of the first six months, the long term provisional or temporary teeth can be replaced by a nicer acrylic prosthesis with plastic or porcelain teeth that has a framework of metal for strength. The new teeth can also be made from porcelain with a metal or zirconia framework. The greater the amount of bone loss, the more difficult to fabricate a metal framework, and a zirconia framework may be better if porcelain is desired.
Porcelain has the advantage of being impervious to stains and wear, but can be harder on the implants unless the opposing teeth are made of acrylic for a cushioning effect.
